What Connect does

The shortest path between people and care

For many families, the biggest barrier isn't the absence of services — it's finding them, understanding them, and using them. Connect turns confusing systems into usable ones.

Health & digital literacy

Plain-language health education and hands-on digital skills: patient portals, telehealth, trustworthy health information, and everyday health technology.

System navigation

Helping individuals and families understand eligibility, find services, coordinate across health and social supports, and follow through on referrals.

Community outreach & education

Workshops, health fairs, faith- and community-based sessions, and multilingual materials designed with the communities they serve.

Patient education & self-advocacy

Preparing people to ask questions, understand their conditions and medications, and take a confident seat in their own care.

Why it matters

Support exists. Access doesn't — yet.

Too many people fall through the gaps between programs — not because help doesn't exist, but because reaching it demands time, digital skills, language, and system knowledge that are unevenly shared. Every unnavigated referral and unread portal message is care that was funded but never delivered.

Connect exists to close that last mile. When people can find, understand, and use the services already built for them, existing investments in health finally reach the people they were meant for — and families gain skills that keep paying off long after a single visit.
